FMLA Paperwork Policy

Oral Surgery Associates is committed to providing timely and accurate completion of medical leave
documentation. To ensure all forms are completed accurately and based on the patient’s medical care, the
following policy applies:

• Processing Time: Please allow five (5) business days for completion of FMLA, disability, and other
  medical leave paperwork. We are unable to accommodate same-day requests.
• Forms Submitted Before Surgery: We will complete pre-operative paperwork to the best of our
  medical knowledge. However, many questions on FMLA and leave certification forms cannot be
  accurately answered until after the procedure has been completed.
• Post-Operative Updates: If additional information is required after surgery, we will complete one
  revised certification once the procedure has been completed and the patient’s recovery can be
  appropriately assessed. Please allow an additional five (5) business days for processing.
• Routine Oral Surgery Procedures: Many oral surgery procedures involve a brief recovery period
  and do not require ongoing medical restrictions. For these routine procedures, our office will
  complete the initial certification and one post-operative revision, if medically appropriate. If
  additional documentation is requested after these two certifications, we will instead provide a work
  excuse letter and any applicable post-operative instructions documenting the anticipated recovery
  period and return-to-work date.
• Medical Certification: Our providers will certify only information that is medically appropriate and
  supported by the patient’s condition, treatment, and expected recovery. We cannot certify
  information that is unknown, speculative, or inconsistent with the patient’s medical care.
  Exceptions to this policy may be made for patients with complications, extended recoveries, or medical
  conditions that require additional documentation at the discretion of the treating surgeon.
